I just came across one of the most attractive contact pages I've ever seen. Their "fill in the blanks" approach feels friendlier than text field boxes, and likely encourages more complete information when people make contact.

I love those - there's a report here http://www.lukew.com/ff/entry.asp?1007 that they increase completion by up to 40% - have wanted to try one for ages but I don't get to design the forms at work :(

Thanks. That fits my style so much better than a form. I have got to keep that in mind.

Really.  More people complete those?  Because if you just need a photographer for, say a family photo, that form woudl turn me off...?
