The quality of all Yoast plugins have risen by a factor of 10 since he hired skilled developers. There are just so many plugin combinations out there that it is impossible for them to catch all the bugs with the limited number of beta testers they have. They do a pretty good job on minor versions but fail at the bigger releases. Usually they have the worst bugs fixed within a week.
After a hack, I have everything set to auto-upgrade and if it brings a site down, I have it in git and I back it out.
This auto-upgraded and no problems yet on my end.
I did get a note about it wanting to recaclulate SEO scores and based on this thread decided not to push that button until another update rolls out.
I had a nightmare moving from v2.xx to the recently release v3.0 and had to role back several times on most sites, leaving a particularly unimportant site to go through all the updates. Everything seems resolved now with 3.1.2
I was friends with Joost when i was a member of a private SEO group on Farcebook (i left FB due to privacy concerns i highlighted), he was forthcoming and hlped me out with an odd title-injection problem. Most of the problems are environment-related and not due to any flaw in the plugin as far as i have seen.
I only use it for it's bulk title/description editing.
