I saw that elsewhere. Seems to be true. And the latest in a long line of near misses. Not counting "intentional" near misses like where the Soviet officer refused a direct order during the Cuban Missile Crisis and did not launch the nukes despite being ordered to do so.

1960: NORAD interprets moonrise over Norway as a massive Soviet attack

1962: soldiers on Okinawa receive the order to launch and choose to ignore it
