a decent Survey Form...

I was just asked about putting up a survey form for a private company survey of management, but I haven't conducted one in years. I was wondering if Th3Core could recommend anything simple, secure, and cheap/free.

I have a really early version of LimeSurvey on a server somewhere, but I really didn't care for that package. Explaining how to use it took longer than the survey itself. I'm not sure I want to bother updating and using that again.

I hear the Google Docs Form feature is pretty simple, but the IT people at this company have already expressed their reservations about using Google due to security concerns. If I could convince them otherwise this sounds like a decent tool.

Then we have all of the 3rd party services like SurveyMonkey, which I know precious little about. Are the free options secure and private?

The survey is nothing special. It's an opinion survey for a magazine. The point where security is involved seems to be that they want to collect the  people's names and contact info of management-types.

Any pointers?

I like surveymonkey. I'd recommend going with the paid option though - not so much for the branding elements but the reporting is better on paid, or was last time I looked at free.

Agreed with Gurtie. Surveymonkey is v.good, but getting your data out of the free version is painful.
